Playing around in iCal just now (see my prior post), I thought of a possible cause of this bug: In iCal, I have two personal calendars ("personal" and "work"), with "personal" being my "preferred" calendar for iPhone-to-iCal syncing (in the iPhone panel in iTunes).
The above 90-day alarm/reminder that didn't go off last week is in my "work" calendar. Perhaps only the alarms/reminders from one's "preferred" iCal calendar go off on iPhone (??).
