You build a list yourself, much like AdBlock in Firefox, if you've ever used that.
You right-click on an image, iframe or flash animation (there's an option in the Safari menu bar to enable flash right-clicking) and specify that you want to block a URL. You can also use wildcards to filter out groups of addresses, such as:
http://*.doubleclick.net/* --> This would block doubleclick.net ads that use a subdomain
http://doubleclick.net/* --> This would block doubleclick.net ads that don't use a subdomain
You can also add addresses manually through Safari's preferences (a new pref pane for Safari Block gets added when you install the software).
Basically, the plugin tells Safari to ignore anything being loaded from the addresses you blocked, effectively erasing ads from any page you visit.
